Task: NEC-94 — https://projectron.nerchure.com/tasks/94

Sessions find work outside their card and had no sanctioned way to file it: one left it in a comment, another wrote its own script and put cards straight into Ready and Planning. card.mjs create now files a follow-up into Backlog only, with a back-link and a read-back; card.mjs queue is the one way into Ready, from Backlog, at a priority the user chose; move --to Ready is refused. The work-queue skill tells sessions to use them and to ask before anything reaches Ready. First use filed NEC-100.

card.mjs create files a follow-up into Backlog, always, with a line linking
back to the card it was found on, and reads the column back rather than
trusting the view's default. card.mjs queue is the one way from Backlog to
Ready, at a priority the user chose; move --to Ready is refused.

Review found three acceptance lines defended only by pure helpers, a queue
read-back no test could fail, and a create that failed after making its card
without saying which card, so a retry would file it twice.
