Part of #77. Needs the ledger (#82).

Why

No record of what we ship exists in any published place. A changelog page is
also the kind of page that earns impressions for the terms a product page never
matches.

What to do

Render docs/changelog-plugin.md through the existing PAGES mechanism in
site/build.mjs — the same path /security, /terms and /privacy take.

Four coordinated edits, and missing the fourth fails silently:

PAGES
SITEMAP
rewriteLinks()'s published map
deploy-site.yml's paths:

That last one is the trap already sprung once: docs/security-model.md built a
page for four days without being able to deploy it, because the path filter was
not updated with the page. The comment there now says to add and remove these
with the pages, in the same commit.

The description must sit inside 100-160 characters — the guard in page()
enforces it in both directions, and will throw at build time otherwise.

The server ledger is a separate decision

docs/changelog-server.md has nowhere to publish yet and is retrospective —
it names what already shipped, not what is coming. If it is published, its
## Unreleased means "live but unnamed" and the page must not imply otherwise.
